Core Responsibilities

Primary Functions

  • Conduct comprehensive intake assessments and needs evaluations for new clients, documenting social, legal, medical, and financial barriers to develop individualized, client-centered service plans that align with organizational goals and client preferences.
  • Provide direct advocacy and representation for clients in interactions with public agencies, courts, healthcare systems, insurers, landlords, and employers to resolve disputes, secure benefits, and remove barriers to stable housing, income, or health services.

  • Develop and implement individualized case plans that coordinate legal referrals, benefits enrollment, housing stabilization, mental health services, and other community resources, tracking milestones and outcomes in the organization's case management system (CRM/EMR).
  • Conduct legal research and prepare documentation, petitions, and evidence packages for administrative hearings or court proceedings, collaborating with legal counsel when necessary to strengthen client cases.
  • Facilitate family or multi-party conferences, restorative practice meetings, and mediation sessions to negotiate resolutions, safety plans, or service agreements that protect client rights and promote long-term stability.
  • Build and maintain collaborative partnerships with community-based organizations, government agencies, healthcare providers, and pro bono attorneys to expand service pathways and streamline referral processes for clients.
  • Deliver trauma-informed and culturally competent direct services, applying sensitivity to race, language, immigration status, disability, and socio-economic context while promoting clients' informed choice and self-determination.
  • Monitor and report case outcomes, social determinants of health, and service utilization metrics, producing weekly and monthly reports that inform program evaluation and continuous quality improvement.
  • Educate clients, families, and community groups about legal rights, benefits eligibility, healthcare access, housing rights, and community resources through one-on-one counseling, workshops, and written materials optimized for accessibility.
  • Prepare detailed client records, case notes, consent forms, and release of information documents that meet confidentiality, HIPAA (if applicable), and organizational compliance standards.
  • Coordinate emergency interventions including safety planning, emergency housing placement, crisis stabilization referral, and linkages to emergency financial assistance programs.

  • Serve as a liaison to employers, vocational services, and education programs to support client employment readiness, reasonable accommodations, workplace advocacy, and job placement efforts.
  • Negotiate with vendors, landlords, and creditors on behalf of clients to secure payment plans, lease accommodations, or eviction prevention measures that preserve housing stability.
  • Maintain up-to-date knowledge of statutes, regulations, and case law relevant to client populations (immigration, family law, housing, public benefits), ensuring advocacy approaches reflect current legal environments.
  • Design and deliver multi-lingual or culturally adapted materials and services in partnership with translation services and cultural brokers to maximize accessibility and client comprehension.

Education & Experience

Educational Background

Minimum Education:

  • Bachelor's degree in Social Work, Human Services, Public Policy, Criminal Justice, Psychology, Paralegal Studies, or related field; OR equivalent combination of education and direct advocacy experience.
