Core Responsibilities
Primary Functions
- Conduct comprehensive pre-anesthesia evaluations for adult, pediatric, and obstetric patients, synthesizing medical history, medication reconciliation, airway assessment, cardiovascular and respiratory status, labs, imaging, and ASA classification to formulate individualized anesthesia plans compliant with facility protocols and ASA guidelines.
- Develop and communicate clear anesthesia plans in coordination with surgeons and procedural teams, selecting the appropriate anesthetic technique (general, MAC, regional, neuraxial, sedation) and contingency strategies for anticipated complications.
- Administer induction and maintenance of general anesthesia, titrating intravenous and inhalational agents to hemodynamic targets while continuously monitoring oxygenation, ventilation, circulatory status, depth of anesthesia, and neuromuscular blockade using standard monitors and processed EEG when indicated.
- Perform advanced airway management including rapid sequence induction (RSI), endotracheal intubation, use of video laryngoscopy, supraglottic airway devices, fiberoptic intubation, and emergency front-of-neck access per institutional airway algorithms.
- Provide regional anesthesia techniques including peripheral nerve blocks, neuraxial anesthesia (spinal, epidural), and local infiltration with ultrasound and nerve stimulator guidance, maintaining sterile technique and monitoring for block efficacy and complications.
- Manage invasive hemodynamic monitoring and vascular access: place and interpret arterial lines, central venous catheters, and titrate vasoactive infusions (vasopressors, inotropes) for perioperative hemodynamic optimization.
- Administer and titrate analgesics, sedatives, muscle relaxants, reversal agents, and emergency medications during anesthesia, ensuring correct dosing, aseptic preparation, verification, and documentation per medication safety protocols.
- Lead safe emergence from anesthesia and extubation decisions, interpret neuromuscular monitoring, reverse blockade appropriately, and ensure airway reflexes and adequate ventilation prior to transfer to PACU or ICU.
- Provide post-anesthesia care including pain control optimization, nausea/vomiting prophylaxis and treatment, hemodynamic stabilization, monitoring for respiratory compromise, and clear handoff to PACU or ICU staff with a focused anesthesia report.
- Recognize and manage anesthesia-related emergencies (anaphylaxis, malignant hyperthermia, airway loss, severe hemorrhage, cardiovascular collapse), lead code responses, activate crisis protocols, and document events for quality and safety review.
- Participate in preoperative optimization and patient education, discussing anesthesia risks and options, obtaining informed consent as delegated, and documenting discussions in the electronic health record (EHR/EPIC/Cerner).
- Implement perioperative multimodal analgesia and enhanced recovery after surgery (ERAS) protocols to reduce opioid use, shorten length of stay, and improve outcomes for ambulatory and inpatient surgical populations.
- Maintain, troubleshoot, and verify anesthesia equipment — anesthesia machines, ventilators, monitoring devices, regional block kits, ultrasound, and emergency carts — performing pre-case checks and reporting equipment malfunctions.
- Monitor fluid management and blood product administration intraoperatively; interpret point-of-care testing (ABGs, glucose, Hgb/Hct) and implement transfusion and resuscitation strategies in collaboration with the surgical team.
- Provide obstetric anesthesia care including labor epidurals, combined spinal-epidural (CSE), and anesthesia for cesarean delivery with attention to fetal-maternal monitoring, neuraxial block considerations, and rapid response for emergent deliveries.
- Deliver pediatric anesthesia care adapted to age-specific physiology and medication dosing, with emphasis on airway management, fluid balance, thermoregulation, and family-centered communication.
- Collaborate in multidisciplinary rounds for complex patients, contribute anesthesia perspectives to perioperative planning for cardiac, neuro, pediatric, trauma, and transplant cases, and coordinate postoperative disposition with admitting services.
- Maintain thorough, timely, and legally compliant anesthesia documentation in the EHR including pre-op assessment, intraoperative events, medication administration records, anesthesia record flowsheets, and post-anesthesia notes to support billing, quality, and regulatory requirements.
- Participate in continuous quality improvement efforts including morbidity and mortality reviews, root cause analyses, adherence to clinical pathways, chart audits, and reporting of adverse events to improve patient safety and outcomes.
- Supervise and precept SRNAs, anesthesia students, residents, and new staff; provide constructive feedback, simulation-based training, competency assessments, and maintain compliance with credentialing and privileging guidelines.
- Ensure compliance with infection control, OSHA, JCAHO, state and federal regulations, and institutional policies related to controlled substances, sharps safety, sterile technique, and hazardous waste handling in perioperative settings.
- Administer sedation and anesthesia outside the OR (interventional radiology, endoscopy, cardiac cath lab, MRI), adapting to remote-site challenges and ensuring patient monitoring and safety standards are met.
- Participate in staffing models, on-call rotations, and rapid response teams to provide continuous anesthesia coverage while balancing workload with workload forecasting and resource allocation.
- Contribute to departmental education by developing protocols, in-service training, simulation drills (malignant hyperthermia, airway emergencies), and evidence-based practice updates to promote a culture of safety and clinical excellence.
Secondary Functions
- Assist with anesthesia service scheduling, case coordination, and throughput initiatives to optimize block utilization and reduce turnover time.
- Support departmental billing and coding accuracy by verifying procedure codes, anesthesia time, and documentation requirements to maximize reimbursement and compliance.
- Lead or contribute to research projects, data collection, and outcomes measurement tied to anesthesia practice, ERAS protocols, or pain management interventions.
- Participate in vendor evaluation and procurement decisions for anesthesia devices, regional block kits, and monitoring equipment when requested by leadership.
- Serve on institutional committees for patient safety, perioperative services, pharmacy & therapeutics, and infection control to represent anesthesia practice and influence policy.
- Mentor peers and junior clinicians by providing case debriefs, evidence summaries, and professional development guidance to improve departmental competency.
- Maintain a current professional portfolio including certifications, continuing education credits, case logs, and competency checklists for privileging and regulatory audits.
- Contribute to community outreach or patient education initiatives about anesthesia options, perioperative preparation, and postoperative pain management where applicable.
