Core Responsibilities

Primary Functions

  • Provide expert preoperative evaluation and risk stratification for adult cardiac surgical patients, including review of cardiac imaging, optimization of medical therapy (antiplatelet/anticoagulant management), and clear informed consent discussions with patients and families tailored to complex cardiac conditions.
  • Perform high-acuity anesthetic induction, airway management, and securement for cardiac surgical cases (CABG, valve surgery, aortic aneurysm/dissection repair), with readiness to manage difficult airway scenarios and hemodynamic instability during induction.
  • Deliver targeted intraoperative hemodynamic management using invasive monitoring (arterial lines, pulmonary artery catheters, central venous catheters) and vasoactive/inotropic titration to optimize end-organ perfusion during critical phases (sternotomy, cannulation, cross-clamp, reperfusion).
  • Interpret and perform transesophageal echocardiography (TEE) (basic and advanced views) to guide surgical decision-making, valve repair assessment, volume/contractility evaluation, and immediate post-bypass cardiac function assessment; document TEE findings per institutional standards.
  • Manage perioperative cardiopulmonary bypass (CPB) coordination with perfusionists and surgical teams: anticipate hemodynamic changes at cannulation/decannulation, communicate flow and temperature goals, and optimize anticoagulation and hemodilution strategies.
  • Lead anesthetic care for complex procedures including redo sternotomies, aortic root/arch surgery, pulmonary thromboendarterectomy, minimally invasive valve procedures, and combined CABG/valve operations with close coordination across teams.
  • Provide perioperative management for patients requiring mechanical circulatory support: initiate/monitor ECMO, evaluate and manage perioperative LVAD/VAD patients, assist with cannulation planning, and contribute to device weaning/optimization strategies in collaboration with the cardiac surgery and perfusion teams.
  • Implement goal-directed transfusion and blood conservation strategies, including use of cell salvage, point-of-care coagulation testing (TEG/ROTEM) interpretation, and coordination with blood bank during massive transfusion protocols.
  • Deliver postoperative critical care: manage ventilator liberation/fast-track extubation protocols, hemodynamic stabilization in the cardiothoracic ICU, pain control tailored to cardiac surgical patients, and transition of care handoffs to intensivists and hospitalists.
  • Provide anesthesia for invasive cardiac catheterization and hybrid procedures (transcatheter aortic valve replacement — TAVR, structural heart interventions), balancing sedation strategies vs general anesthesia and using TEE and fluoroscopic correlation.
  • Respond to intraoperative and postoperative emergencies (cardiac arrest, arrhythmia management, massive hemorrhage, coronary graft failure) with advanced cardiac life support (ACLS) protocols and immediate resuscitative interventions.
  • Maintain meticulous perioperative documentation and billing compliance for anesthesia services, TEE procedures (when applicable), and critical care billing codes; ensure accurate capture of critical intraoperative events and case time.
  • Serve as attending anesthesiologist for teaching and supervision of residents, fellows and CRNAs; deliver structured bedside education, simulations, and formal lectures to foster a high-quality training environment.
  • Participate in multidisciplinary pre-op huddles and surgical planning conferences (e.g., heart team meetings for TAVR, transplant, LVAD) to align perioperative strategy and patient selection.
  • Lead and contribute to quality improvement and patient safety initiatives specific to cardiac anesthesia: morbidity/mortality reviews, protocol development for transfusion, anticoagulation reversal, infection prevention, and enhanced recovery after cardiac surgery (ERACS) pathways.
  • Collaborate with cardiology, surgical teams, perfusion, nursing, respiratory therapy, and pharmacy to develop and standardize perioperative protocols for hemodynamic management, anticoagulation, and mechanical support.
  • Participate in institutional efforts for stroke, renal protection, and delirium prevention in cardiac surgical populations — implement evidence-based protocols and monitor outcomes.
  • Maintain readiness for on-call responsibilities including emergent cardiac surgery, ECMO cannulation, and transplant operations; provide timely clinical decision-making and remote support as needed.
  • Lead or participate in clinical research, outcomes analysis, and registry contributions (STS, institutional databases) to advance perioperative cardiac care and support scholarly productivity.
  • Supervise and mentor perioperative staff in the safe use of cardiac anesthesia equipment (TEE probes, invasive monitors, pump consoles), troubleshoot perioperative technology failures, and ensure equipment maintenance and competency.
  • Engage in patient- and family-centered communication: explain anesthetic plans, expected ICU course, potential complications, and engage in shared decision-making for high-risk cardiac procedures.
  • Contribute to credentialing and privileging processes within the department (TEE, ECMO, cardiac anesthesiology privileges) and maintain CME and certification requirements.
  • Evaluate and implement enhanced recovery and multimodal analgesia strategies for cardiac surgical patients, aiming to reduce opioid use, shorten ICU/hospital length of stay and improve functional outcomes.
  • Coordinate perioperative anticoagulation and antiplatelet strategies with cardiology and surgery for patients with recent stents, mechanical valves, or therapeutic anticoagulation requirements.

Secondary Functions

  • Champion departmental quality metrics and participate in institutional committees for perioperative services, cardiac surgical outcomes and patient safety.
  • Support clinical pathways development for fast-track extubation, ERAS for cardiac surgery and standardized transfusion/TEG algorithms.
  • Mentor junior faculty, provide peer review and help maintain departmental clinical standards and protocols.
  • Assist in scheduling, staffing allocation and on-call rotation planning for the cardiac anesthesia team.
  • Engage in community and patient education efforts related to perioperative cardiac risk reduction and procedural preparation.
  • Participate in equipment procurement reviews, TEE/ultrasound device evaluations and protocol-based workflow optimization.
  • Contribute to institutional emergency preparedness planning for mass cardiac surgical demands or ECMO surge scenarios.
  • Help develop billing templates and documentation best practices to optimize reimbursement for cardiac anesthesia and critical care services.
