  • Verify physician orders and patient identification prior to specimen collection, ensuring 2-point identification in accordance with institutional policies and preventing mislabeling or wrong-patient errors.
  • Accurately label all specimens at the bedside or point of collection with specimen type, collection date/time, collector initials, and requisition identifiers to maintain chain of custody and laboratory sample integrity.
  • Follow proper specimen handling procedures (temperature control, light sensitivity, anticoagulant mixing, timely centrifugation) to maintain specimen viability for accurate downstream testing.
  • Enter and update specimen collection, order verification, and patient information into the electronic health record (EHR) and laboratory information system (LIS) with precise documentation of collection time, technique, and any difficulties encountered.
  • Triage stat, routine, and timed collections (e.g., glucose tolerance tests, therapeutic drug monitoring) and coordinate rapid transport to the laboratory to meet turnaround time (TAT) expectations.
  • Maintain rigorous infection prevention and bloodborne pathogen control practices, including correct use of personal protective equipment (PPE), proper sharps disposal, and compliance with OSHA and facility policies.
  • Obtain informed consent and explain procedures in a clear, compassionate manner tailored to patients’ language, age, cognitive ability, and cultural considerations while respecting patient privacy and HIPAA requirements.
  • Perform point-of-care testing (POCT) and quality control checks as assigned (e.g., glucose monitoring), document results, and escalate abnormal or critical values per protocol.
  • Assist with venous access troubleshooting and escalation for difficult draws, using vein selection strategies and adjunctive devices (warm compress, tourniquet technique, vein transillumination) and documenting attempts per facility policy.
  • Maintain phlebotomy carts and mobile draw stations, ensuring supplies are fully stocked, organized, not expired, and inventoried using first-in-first-out (FIFO) methods.
  • Prepare and ship specimens requiring special handling or courier services, including frozen or biohazard-labeled samples, following packaging, documentation, and DOT/IATA guidelines when applicable.
  • Participate in quality assurance (QA) and quality improvement (QI) initiatives by documenting non-conformances, contributing to root cause analyses for specimen rejection, and implementing corrective actions to reduce pre-analytical errors.
  • Train and mentor new phlebotomy hires, students, and cross-trained staff in standard collection procedures, specimen labeling, and patient communication techniques; assist with competency assessments and skills validation.
  • Communicate effectively with nurses, physicians, lab technologists, and front-desk staff to resolve order discrepancies, specimen rejection causes, and to prioritize urgent collections during peak volumes.
  • Respond professionally to patient complaints, adverse reactions (e.g., syncope, hematoma), and emergency situations during collection; initiate appropriate first-aid measures and escalate to clinical teams as needed.
  • Adhere to institutional policies and federal/state regulations for documentation retention, specimen retention times, and patient confidentiality, ensuring all actions meet accreditation (CLIA/CAP) standards.
  • Calibrate, clean, and perform basic maintenance on point-of-care and collection equipment (centrifuges, tube mixers, collection chairs) and promptly report malfunctioning devices to biomedical/engineering.
  • Monitor and report inventory usage, reorder thresholds for collection supplies and PPE, and collaborate with supply chain to minimize stockouts that could impact clinical operations.
  • Participate in cross-functional committees (safety, infection control, specimen management) to provide frontline insights that improve operational efficiency and patient safety.

Secondary Functions

  • Assist laboratory staff with specimen accessioning or specimen preparation when workload demands require cross-coverage.
  • Support outreach phlebotomy or mobile collection programs by helping schedule community drives and performing onsite collections at outreach locations.
  • Document and escalate patterns of frequent specimen rejection or mislabeled specimens, contributing to department training materials and refresher sessions.
  • Aid in onboarding and competency documentation for new hires by serving as a subject-matter resource for hands-on phlebotomy skills.
  • Help maintain compliance documentation for audits, including vaccination records, TB screening, background checks, and ongoing competency logs.
  • Provide input to leadership on scheduling improvements and staffing models to optimize coverage for peak draw times and reduce patient wait times.
  • Participate in disaster response or surge staffing plans that require rapid redeployment of phlebotomy resources across clinical areas.
  • Assist with periodic review and updates of phlebotomy procedure manuals and job aids to reflect current best practices and regulatory changes.
  • Support patient education efforts by providing clear post-draw care instructions and resources for abnormal results follow-up.
  • Contribute to a positive patient experience by consistently demonstrating professionalism, punctuality, and a service-oriented attitude.
