Core Responsibilities
Primary Functions
- Conduct comprehensive, systematic patient assessments (vital signs, neurological, respiratory, cardiac, pain, psychosocial) and document findings accurately in the Electronic Health Record (EHR) to guide clinical decision-making and care planning.
- Develop, implement, and revise individualized nursing care plans based on assessments, physician orders, clinical pathways, and evidence-based protocols to optimize patient outcomes.

- Coordinate multidisciplinary care by collaborating with physicians, therapists, pharmacists, social workers, case managers, and community resources to facilitate safe transitions of care and timely discharge planning.
