Core Responsibilities
Primary Functions
- Perform initial nutrition screening for new admissions by collecting weight, height, BMI, allergy history, current oral intake patterns and pertinent medical history, and escalate identified malnutrition risk to the registered dietitian for assessment and intervention.
- Accurately assemble and deliver patient meal trays according to physician orders and registered dietitian plans, verifying diet orders, allergies, texture modifications and meal substitutions at the point of service to ensure patient safety.
- Prepare and portion therapeutic diets and modified-consistency foods in compliance with standardized recipes and diet manual instructions, ensuring adherence to physician-prescribed calorie, sodium, carbohydrate, and fluid restrictions.
- Document patient meal consumption, percentage of meal eaten, and bedside feeding assistance provided in the electronic medical record (EMR) and report notable changes in intake or tolerance to the dietitian and nursing staff.
- Assist the clinical nutrition team with tube-feeding and enteral nutrition support under supervision, including preparation of formula, tube flushes, and accurate labeling and documenting of enteral feeds per policy and sterile technique requirements.
- Conduct ongoing tray audits and meal verification checks to uphold quality standards, accuracy of therapeutic diets and portion control, and to capture data used for foodservice quality improvement initiatives.
- Reconcile diet orders with pharmacy and nursing when parenteral/enteral orders conflict, and escalate discrepancies to ensure coordinated nutrition care and patient safety.
- Support the dietitian in outpatient and inpatient nutrition education by preparing patient handouts, scheduling follow-up sessions, and reinforcing basic nutrition guidance at the point of care.
- Assist with routine nutrition screenings and monitoring programs such as malnutrition risk assessments, calorie counts, and food/fluid intake tracking to support interdisciplinary care planning.
- Maintain and sanitize food prep areas, service counters, carts and equipment according to HACCP, ServSafe and facility infection control policies, including daily cleaning logs and temperature documentation.
- Receive, inspect and store food and nutrition supplies, rotate inventory by FIFO, monitor stock levels, and prepare purchase requests or communicate shortages to the nutrition manager for timely replenishment.
- Support menu cycle planning and recipe standardization by providing feedback on patient acceptability, portion sizes and preparation efficiency to the dietitian and foodservice team.
- Verify and code diet orders in the EMR and/or food service management system, ensuring accurate diet designations for kitchen production and clinical documentation for billing and compliance.
- Provide bedside feeding assistance for patients who require help with eating, ensuring dignity and safety while documenting level of assistance and any observed swallowing or aspiration concerns for speech therapy and dietitian follow-up.
- Participate in interdisciplinary rounds and care conferences to provide nutrition-related observations, share intake data, and help implement dietitian-directed care plans.
- Train and mentor new dietary staff on therapeutic diet requirements, tray assembly procedures, allergen safeguards and EMR meal documentation standards to maintain consistent service quality.
- Execute special event and therapeutic meal preparation such as renal-friendly menus, diabetic-friendly menus, dysphagia diets and cardiac diet options during increased patient volumes or program needs.
- Compile and prepare nutrition-related charts, reports and compliance documentation for internal audits, regulatory surveys (e.g., Joint Commission), and quality improvement projects led by the nutrition services team.
- Support research or pilot nutrition programs by collecting intake data, assisting with patient recruitment logistics, and maintaining secure and accurate records in accordance with HIPAA and research protocols.
- Assist in patient and family nutrition counseling sessions by corroborating recorded intake histories, clarifying meal patterns, and reinforcing evidence-based education as directed by the registered dietitian.
- Monitor and document meal delivery timelines and service metrics, collaborate on workflow improvements to reduce tray delays and waste, and contribute to cost-containment strategies for the nutrition department.

Minimum Education:
