Core Responsibilities

Primary Functions

  • Perform comprehensive 2-D, M-Mode, and Doppler echocardiograms, including transthoracic (TTE) studies, to acquire high-quality diagnostic images of the heart's chambers, valves, and vessels.
  • Collaborate with cardiologists and nurses during stress echocardiography procedures (both exercise and pharmacological), acquiring and comparing images at rest and peak stress to assess cardiac function under load.
  • Assist cardiologists in the performance of Transesophageal Echocardiography (TEE), preparing the patient, operating the ultrasound console, and ensuring the integrity of the TEE probe.
  • Utilize advanced techniques such as strain imaging (speckle tracking) and 3D/4D echocardiography to provide quantitative analysis of myocardial function and complex anatomical structures.
  • Accurately measure cardiac structures and blood flow velocities, performing detailed calculations of valvular function, ejection fraction, and diastolic/systolic function according to established protocols and ASE guidelines.
  • Prepare patients for procedures by explaining the process in a clear and compassionate manner, verifying patient identification, obtaining informed consent, and ensuring their comfort and safety throughout the examination.
  • Analyze the acquired sonographic data in real-time, identifying anatomical and physiological abnormalities and tailoring the exam to provide the most comprehensive diagnostic information possible.
  • Generate concise, accurate preliminary reports in the reporting system (PACS/EMR), summarizing findings, measurements, and any identified critical results for the interpreting cardiologist's review.
  • Immediately notify the interpreting physician or referring provider of any critical findings, such as significant pericardial effusion, suspected cardiac thrombus, or severe valvular dysfunction.
  • Operate and troubleshoot complex ultrasound equipment, making adjustments to image quality and system parameters to optimize diagnostic output for each patient's unique anatomy.
  • Maintain a sterile and safe environment, particularly during invasive procedures like TEEs and bubble studies, adhering to strict infection control policies.
  • Prepare and administer ultrasound contrast agents (e.g., Definity, Lumason) and agitated saline for bubble studies as per physician order and departmental protocol to enhance image quality and diagnostic accuracy.
  • Evaluate prosthetic valve function, assessing for proper seating, mobility, and identifying any potential stenosis, regurgitation, or perivalvular leaks.
  • Perform specialized pediatric and neonatal echocardiograms to diagnose and monitor congenital heart defects, adapting techniques for smaller patients.
  • Participate in the departmental call rotation to provide emergent echocardiography services for after-hours, weekend, and holiday coverage.
  • Document all procedures, patient information, and technical findings accurately and contemporaneously within the patient's Electronic Medical Record (EMR).
  • Ensure all imaging studies are correctly archived to the Picture Archiving and Communication System (PACS) for review and long-term storage.
  • Adhere to all institutional and departmental policies, procedures, and safety standards, including HIPAA regulations for patient privacy.
  • Perform echocardiograms in various clinical settings, including the outpatient lab, inpatient rooms, intensive care units (ICU), and the emergency department, adapting to different environments and patient acuity levels.
  • Provide guidance and support to students, residents, and new staff members, acting as a clinical resource for proper scanning techniques and protocols.
  • Stay current with advancements in echocardiography technology, techniques, and clinical guidelines through continuous education and professional development activities.

Secondary Functions

  • Perform routine maintenance, calibration, and cleaning of ultrasound equipment and probes to ensure optimal performance and longevity.
  • Participate in departmental quality assurance and continuous quality improvement (CQI) initiatives, including peer review of studies and process improvement projects.
  • Assist in managing inventory of essential supplies, such as gels, contrast agents, and sterile procedure kits, and notify leadership of ordering needs.
  • Contribute to the accreditation process for the echocardiography lab (e.g., Intersocietal Accreditation Commission - IAC) by helping maintain records and adhere to standards.
