Core Responsibilities

Primary Functions

  • Performs comprehensive and high-quality transthoracic echocardiograms (TTE), including 2D, M-Mode, and Doppler imaging, to accurately assess cardiac anatomy, chamber sizes, and ventricular function.
  • Assists cardiologists in the performance of Transesophageal Echocardiograms (TEE), preparing the patient, operating the ultrasound equipment, and acquiring optimal images during the procedure.
  • Conducts pharmacological and exercise stress echocardiograms, monitoring patient vital signs and acquiring images at rest and peak stages to evaluate for ischemia and valvular dysfunction.
  • Utilizes advanced imaging techniques, such as strain imaging (speckle tracking) and 3D/4D echocardiography, to provide quantitative analysis of myocardial function and complex structural heart disease.
  • Administers ultrasound contrast agents (e.g., Definity, Lumason) as per protocol to enhance endocardial border delineation and improve diagnostic accuracy in technically difficult studies.
  • Performs specialized procedures such as bubble studies to detect intracardiac shunts and assesses prosthetic heart valve function for stenosis or regurgitation.
  • Prepares preliminary reports by accurately measuring and calculating cardiac parameters, including ejection fraction, valvular gradients, and diastolic function, for physician review.
  • Enters all patient data and preliminary findings into the Picture Archiving and Communication System (PACS) and Electronic Medical Record (EMR) with meticulous attention to detail.
  • Analyzes the patient's clinical history and presenting symptoms to tailor the echocardiographic examination and ensure all relevant clinical questions are addressed.
  • Explains procedures to patients to reduce anxiety and gain their cooperation, ensuring a high standard of patient care and comfort throughout the examination.
  • Operates and maintains ultrasound equipment, performing routine cleaning, quality control checks, and minor troubleshooting to ensure optimal performance and safety.
  • Adheres to established protocols and standards set by the Intersocietal Accreditation Commission (IAC) to maintain lab accreditation and ensure consistent quality.
  • Collaborates directly with cardiologists and referring physicians to review findings, clarify complex cases, and provide technical insights for an integrated diagnostic approach.
  • Recognizes and immediately reports critical findings, such as severe valve dysfunction, pericardial effusion with tamponade physiology, or wall motion abnormalities, to the interpreting physician.
  • Manages patient workflow efficiently to minimize wait times and maximize the number of studies performed without compromising quality.
  • Participates in a rotating call schedule for emergent after-hours and weekend echocardiograms as required by the department.
  • Provides guidance and mentorship to sonography students, interns, or new technologists, fostering a supportive learning environment.
  • Stays current with advancements in echocardiography techniques, technologies, and clinical guidelines through continuous professional development.
  • Ensures a safe patient care environment, adhering to all infection control, radiation safety (if applicable), and institutional safety policies.
  • Evaluates image quality during the examination and makes necessary technical adjustments to transducers, machine settings, and patient positioning to produce optimal diagnostic images.

Secondary Functions

  • Assists in the compilation of statistical data and participates in departmental quality assurance and improvement initiatives.
  • Participates in the evaluation and implementation of new cardiac imaging technologies, protocols, and software updates.
  • Collaborates with cardiologists and other medical staff to facilitate integrated patient care and optimize diagnostic outcomes.

Required Skills & Competencies

Hard Skills (Technical)

  • Transthoracic Echocardiography (TTE): Mastery of standard 2D, M-mode, and Doppler imaging from all acoustic windows.
  • Transesophageal Echocardiography (TEE): Proficiency in assisting with TEE procedures and acquiring high-quality images.
  • Stress Echocardiography: Experience with both exercise and dobutamine stress echo protocols, including image acquisition and wall motion analysis.
  • Doppler Physics & Hemodynamics: Strong understanding and application of Pulsed Wave, Continuous Wave, and Color Flow Doppler to assess blood flow and pressures.
  • Advanced Techniques: Competency in 3D/4D echocardiography and Strain Imaging (Global Longitudinal Strain - GLS).
  • Contrast Agent Administration: Knowledge of and certification for administering ultrasound enhancing agents.

Education & Experience

Educational Background

Minimum Education:

  • Graduate of a certificate or Associate of Science degree program in Diagnostic Medical Sonography accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP).

Preferred Education:

  • Bachelor of Science in Diagnostic Medical Sonography or a related health science field.

Relevant Fields of Study:

Experience Requirements

Typical Experience Range:

  • 0-2 years of clinical experience for an entry-level position. 3-5+ years of experience for senior or specialized roles.

Preferred:

  • Active credentialing as a Registered Diagnostic Cardiac Sonographer (RDCS) from ARDMS or Registered Cardiac Sonographer (RCS) from CCI is required.
  • Basic Life Support (BLS) certification is mandatory; Advanced Cardiac Life Support (ACLS) is often preferred.
  • Prior experience in an acute care hospital or high-volume cardiology clinic setting is highly desirable.
  • Experience with both adult and pediatric patient populations is a significant plus.
