Core Responsibilities
Primary Functions
- Perform comprehensive endodontic examinations, including history, pulp vitality testing, periodontal assessment and occlusal analysis, to determine appropriate nonsurgical or surgical treatment plans.
- Deliver advanced nonsurgical root canal therapy on anterior, premolar, and molar teeth using contemporary techniques (rotary NiTi instrumentation, reciprocating systems, irrigation protocols, and warm vertical obturation) to achieve predictable endodontic outcomes.
- Plan and perform endodontic microsurgery (apicoectomy, root-end resection and retrograde filling) when indicated, utilizing surgical microscopes, micro-instruments, and CBCT guidance for precision.
- Interpret diagnostic imaging including periapical radiographs and cone-beam computed tomography (CBCT) scans to assess complex anatomy, resorption, fractures, and periapical pathology prior to treatment.
- Manage endodontic emergencies and dental trauma (avulsions, luxations, complicated crown-root fractures), providing urgent stabilization, pulpal therapy, splinting, and appropriate follow-up care.
- Diagnose and treat internal and external root resorption, persistent or chronic apical periodontitis, and retreat failed root canal treatments with retreatment protocols and adjunctive therapies.
- Provide isolation with rubber dam for every endodontic procedure and maintain strict adherence to infection control standards (OSHA, CDC, state regulations) and aseptic technique.
- Administer local anesthesia and intra-operative sedation techniques safely and effectively to ensure adequate analgesia and patient comfort during complex procedures.
- Utilize the dental operating microscope or high-magnification loupes for enhanced visualization during canal negotiation, locating accessory canals, and microsurgical procedures.
- Employ adjunctive technologies—ultrasonics for canal refinement, irrigant activation (sonic/ultrasonic/laser), and bioceramic materials for sealing and perforation repair—to optimize treatment predictability.
- Create clear, evidence-based treatment plans and informed consent documentation that explain options, risks, alternatives, prognosis, and estimated costs to patients and referring clinicians.
- Coordinate care with referring general dentists and restorative specialists to plan restorative sequencing, post and core placement, and final prosthetic rehabilitation following root canal therapy.
- Maintain accurate, timely clinical documentation in the practice management system and electronic health record (EHR), including procedure details, materials used, X-ray and CBCT findings, and follow-up instructions.
- Evaluate and manage complications such as instrument separation, canal perforation, persistent infection, or post-operative pain, determining appropriate remediation including surgery or referral.
- Implement and monitor outcome tracking, recall appointments, and radiographic follow-up schedules to assess healing and long-term success rates.
- Counsel patients on post-operative care, pain management, antibiotic stewardship, and oral hygiene practices to support healing and reduce risk of reinfection.
- Triage and manage referral network communications—accepting new referral cases, providing timely consult reports, and offering continuing education or case-based guidance to referring dentists.
- Participate in quality assurance and morbidity/mortality reviews, adverse event reporting, and continuous improvement initiatives focused on clinical outcomes and patient safety.
- Maintain inventory and oversight of endodontic instruments and materials (rotary files, obturation systems, endodontic sealer, root-end filling materials), ensuring availability, sterilization, and equipment maintenance.
- Provide mentorship and supervision to dental assistants and associates in endodontic protocols, rubber dam placement, instrumentation workflow, and clinic safety standards.
- Ensure compliance with billing, coding and documentation standards for endodontic procedures (CDT codes), working closely with administrative staff to optimize claims submission and reimbursement.
- Deliver patient-centered care with sensitivity to dental anxiety and special needs populations, implementing behavioral management techniques and sedation coordination when necessary.
- Contribute to practice marketing and patient education initiatives that explain the benefits of endodontic treatment, tooth retention, and long-term oral health preservation.
- Lead or participate in clinical research, case reporting, quality initiatives, or continuing education programs to remain current with innovations in endodontics and evidence-based practice.
- Maintain licensure, specialty board certification (if applicable), and meet continuing education requirements for endodontic practice and sedation/antibiotic stewardship.
Secondary Functions
- Support clinic scheduling and capacity planning by providing availability and case complexity estimates to front-desk staff and referring offices.
- Assist in onboarding and training new clinical staff on endodontic-specific workflows, instrument care, and radiography protocols.
- Collaborate with clinical leadership to evaluate and recommend investments in endodontic technology (CBCT upgrades, microscopes, apex locators) to improve efficiency and outcomes.
- Participate in interdisciplinary case conferences with prosthodontists, oral surgeons, and orthodontists to plan comprehensive restorative or surgical care.
- Serve as a clinical resource for patient education materials, procedure consent templates, and frequently asked questions about root canal therapy.
- Engage in community outreach or professional networking activities to strengthen referral relationships and promote the specialty service line.
- Help maintain compliance with state dental board regulations, OSHA/CDC infection control audits, and internal credentialing processes.
- Provide on-call coverage for urgent endodontic cases per practice policy, ensuring continuity of care and appropriate triage.
- Contribute to clinical performance metrics, including treatment acceptance rates, success rates, patient satisfaction scores, and operational KPIs.
- Participate in regular peer review, morbidity reviews, and clinical governance meetings to uphold high standards of care.
