Core Responsibilities

Primary Functions

  • Perform comprehensive physical examinations and evidence-based health assessments for patients of all ages, from infants to seniors.
  • Diagnose, treat, and manage a wide range of acute illnesses, injuries, and chronic health conditions, such as diabetes, hypertension, and asthma.
  • Develop, implement, and meticulously evaluate individualized patient care plans that are patient-centered and culturally sensitive.
  • Order, perform, and interpret diagnostic tests, including laboratory work, EKGs, and imaging studies, to inform patient diagnosis and treatment.
  • Exercise prescriptive authority to prescribe medications, therapeutic devices, and other treatments in accordance with state regulations and evidence-based guidelines.
  • Provide extensive patient and family education on disease prevention, health maintenance, chronic disease management, and complex treatment plans.
  • Perform minor in-office procedures, such as suturing, incision and drainage, skin biopsies, and joint injections, as credentialed.
  • Act as the primary point of contact for a panel of patients, ensuring continuity of care and fostering long-term, trusting provider-patient relationships.
  • Promote wellness and disease prevention through routine health screenings, immunizations, and proactive health and lifestyle counseling.
  • Manage and coordinate patient care, including facilitating timely referrals to physicians, specialists, and community resources when necessary.
  • Document all patient encounters, diagnoses, treatments, and care plans accurately and promptly within the Electronic Health Record (EHR) system.
  • Assess and manage the psychosocial, emotional, and behavioral health needs of patients, providing counseling and referrals as appropriate.
  • Respond effectively to in-clinic medical emergencies, providing immediate and appropriate care until emergency services arrive.
  • Monitor patient outcomes and treatment efficacy, adjusting care plans as needed to achieve optimal health and wellness.

Secondary Functions

  • Engage in continuous professional development and maintain up-to-date knowledge of clinical best practices, pharmacology, and healthcare trends.
  • Participate actively in peer reviews, quality improvement initiatives, and clinical case conferences to enhance the quality of care across the organization.
  • Mentor and precept Nurse Practitioner students, nursing students, and other junior clinical staff, fostering a culture of learning and growth.
  • Collaborate with clinic leadership and interdisciplinary teams to develop and refine clinical protocols and patient care workflows.
  • Contribute to population health management initiatives by analyzing patient data to identify trends and address health disparities within the community.
  • Serve as a clinical resource and subject matter expert for registered nurses and medical assistants on the team.
  • Ensure all clinical activities and documentation adhere to institutional policies and federal/state regulations, including HIPAA.

Education & Experience

Educational Background

Minimum Education:

  • Master of Science in Nursing (MSN) from an accredited Family Nurse Practitioner program.
  • Current, unrestricted Registered Nurse (RN) license and Advanced Practice Registered Nurse (APRN) license in the state of practice.
  • National board certification as a Family Nurse Practitioner (FNP-C or FNP-BC).
  • Current DEA license for prescriptive authority.

Preferred Education:

  • Doctor of Nursing Practice (DNP) from an accredited institution.

Experience Requirements

Typical Experience Range: 2-5+ years of FNP experience in a primary care, family medicine, or ambulatory care setting.

Preferred: While we value experience, we strongly encourage motivated new graduates with robust clinical rotation experience and a passion for primary care to apply. Experience working with diverse, underserved patient populations and fluency in a second language (e.g., Spanish) are highly desirable.
