Core Responsibilities
Primary Functions
- Conduct comprehensive, age-appropriate health assessments, including detailed medical histories and head-to-toe physical examinations for diverse patient populations.
- Independently diagnose and manage acute episodic illnesses, injuries, and infections, such as upper respiratory infections, UTIs, and minor dermatological conditions.
- Develop evidence-based treatment plans for the ongoing management of complex and chronic health conditions, including diabetes, hypertension, asthma, and heart disease.
- Order, perform, and critically interpret a wide range of diagnostic and laboratory tests, including blood panels, urinalysis, radiologic imaging, and EKGs, to inform diagnoses.
- Utilize prescriptive authority to prescribe and manage medications, therapeutic devices, and other treatments based on clinical practice guidelines and patient-specific factors.
- Perform a variety of in-office minor surgical procedures, such as suturing of lacerations, incision and drainage of abscesses, skin biopsies, and comprehensive wound care.
- Deliver patient-centered health education and counseling to individuals and families regarding disease prevention, wellness strategies, and chronic disease self-management.
- Administer and advocate for preventative health services, including immunizations, cancer screenings, and annual wellness visits, to promote long-term health.
- Formulate and implement individualized patient care plans in collaboration with the patient, their family, and the broader healthcare team to ensure holistic care.
- Monitor patient responses to treatments and therapies, making necessary adjustments to the care plan to optimize health outcomes and ensure patient safety.
- Facilitate appropriate and timely referrals to medical specialists, mental health professionals, and community-based resources to ensure continuity and coordination of care.
- Maintain meticulous, accurate, and confidential patient records using the Electronic Health Record (EHR) system in a timely and compliant manner.
- Serve as the primary point of contact for a dedicated panel of patients, fostering therapeutic relationships built on trust, respect, and continuity.
- Assess, diagnose, and manage common mental and behavioral health issues within the scope of primary care, such as anxiety, depression, and ADHD.
- Provide comprehensive well-child care, including developmental screenings, anticipatory guidance, and management of common pediatric illnesses.
- Manage women's health needs, including performing Pap smears, providing contraceptive counseling, and managing menopausal symptoms.
- Evaluate and treat non-operative musculoskeletal injuries and conditions, providing guidance on physical therapy and pain management strategies.
- Respond efficiently and compassionately to patient communications, including phone calls, secure messages through the patient portal, and requests for prescription refills.
- Act as a clinical leader and resource, providing guidance, training, and mentorship to registered nurses, medical assistants, and nursing students.
- Engage in patient advocacy, navigating the healthcare system to ensure patients receive the necessary care, resources, and support.

Required Skills & Competencies
Hard Skills (Technical)
- EHR/EMR Proficiency: Expertise in using electronic health record systems (e.g., Epic, Cerner, AthenaHealth) for charting, ordering, and patient management.

Educational Background
Minimum Education:
- Master of Science in Nursing (MSN) from an accredited university.

- National board certification as a Family Nurse Practitioner (FNP-C or FNP-BC).
