Core Responsibilities

Primary Functions

  • Provide comprehensive, continuous, and patient-centered primary care services to individuals and families across all ages, genders, and stages of life.
  • Conduct thorough patient examinations, including well-child visits, annual physicals, and preventative health screenings, to assess and maintain overall health.
  • Diagnose and effectively manage a broad range of acute illnesses, injuries, and complex chronic conditions such as diabetes, hypertension, and heart disease.
  • Develop, implement, and meticulously monitor individualized, evidence-based treatment plans, incorporating pharmacological and non-pharmacological therapies.
  • Order, perform, and critically interpret diagnostic tests, including laboratory work, imaging studies, and EKGs, to inform accurate diagnoses and treatment strategies.
  • Perform a variety of in-office minor surgical and medical procedures, such as suturing, lesion removals, joint injections, and IUD insertions, based on training and credentialing.
  • Maintain precise, comprehensive, and timely documentation of all patient encounters, diagnoses, treatments, and communications within the Electronic Medical Record (EMR) system.
  • Prescribe and manage patient medications, ensuring appropriateness, monitoring for efficacy and side effects, and educating patients on proper usage.
  • Provide extensive patient education on disease prevention, health maintenance strategies, lifestyle modifications, and wellness to empower patients in their own care.
  • Collaborate effectively with a multidisciplinary team of specialists, nurses, pharmacists, and other healthcare professionals to ensure coordinated and holistic patient care.
  • Manage a dedicated patient panel, ensuring continuity of care through proactive follow-ups, chronic care management, and accessible communication.
  • Address the psychosocial needs of patients, providing compassionate counseling and making appropriate referrals to mental health and social support services.

Secondary Functions

  • Participate in a shared on-call rotation to provide after-hours clinical guidance and urgent medical support to patients via telephone.
  • Actively engage in peer review processes, quality assurance programs, and continuous performance improvement initiatives to elevate the standard of care.
  • Provide clinical supervision, mentorship, and education to medical students, residents, and Advanced Practice Providers (APPs) rotating through the practice.
  • Stay current with the latest medical research, clinical guidelines, and advancements in family medicine through dedicated Continuing Medical Education (CME).
  • Efficiently manage administrative duties associated with patient care, including reviewing lab results, responding to patient portal messages, and completing forms.
  • Contribute to community health and wellness by participating in local health fairs, educational seminars, and outreach programs.
  • Adhere strictly to all federal, state, and organizational policies, including HIPAA for patient confidentiality and OSHA for workplace safety.

Required Skills & Competencies

Hard Skills (Technical)

  • Board Certification/Eligibility: Certified or eligible for certification by the American Board of Family Medicine (ABFM) or the American Osteopathic Board of Family Physicians (AOBFP).
  • State Medical Licensure: Possession of an active, unrestricted medical license to practice in the state.
  • DEA Registration: A valid, unrestricted DEA registration to prescribe controlled substances.
  • Advanced Clinical Diagnosis: Superior ability to synthesize patient history, physical findings, and diagnostic data to formulate accurate differential diagnoses.
  • EMR Proficiency: High level of competence using Electronic Medical Record (EMR) systems (e.g., Epic, Cerner, eClinicalWorks) for charting, ordering, and communication.
  • Procedural Competence: Demonstrated skill in performing common outpatient procedures relevant to family medicine.

Education & Experience

Educational Background

Minimum Education:

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school.

  • Current Board Certification in Family Medicine.
  • Additional certifications or training in areas such as Sports Medicine, Geriatrics, or Palliative Care are a plus.

Relevant Fields of Study:

Experience Requirements

Typical Experience Range: 0-5+ years of post-residency clinical experience. New graduates are strongly encouraged to apply!

Preferred:

  • 1-2+ years of experience practicing in an outpatient primary care, community health, or similar ambulatory setting is highly desirable.
  • Experience working with diverse and underserved patient populations is a significant asset.
