Core Responsibilities

Primary Functions

  • Lead the design and execution of comprehensive Community Health Needs Assessments (CHNA) to identify key health priorities, service gaps, and underlying social determinants of health.
  • Analyze and interpret complex quantitative and qualitative data from various sources (e.g., epidemiological, demographic, clinical) to identify health trends, disparities, and priorities.
  • Develop, implement, and monitor long-range, evidence-based strategic health plans and implementation strategies in collaboration with community partners.
  • Evaluate the effectiveness, accessibility, and quality of health services and public health programs using established performance metrics and evaluation frameworks.
  • Author and co-author compelling grant proposals, applications, and reports to secure federal, state, and foundational funding for new and existing health initiatives.
  • Facilitate productive meetings, workshops, and forums with a wide range of stakeholders, including government officials, healthcare providers, community leaders, and the public.
  • Cultivate and maintain strong, collaborative relationships with community-based organizations, coalitions, and public/private partners to advance shared health goals.
  • Research and synthesize scientific literature, best practices, and innovative models to inform program design, policy development, and strategic recommendations.
  • Prepare and deliver clear, persuasive reports, policy briefs, and presentations for diverse audiences, including executive leadership, advisory boards, and community members.
