A Histologist (also called Histotechnologist/Histotech) prepares tissue specimens for microscopic examination and diagnostic testing in clinical and research pathology laboratories. The Histologist performs specimen accessioning, tissue processing, embedding, microtomy, staining (H&E and special stains), immunohistochemistry (IHC), frozen section preparation, slide labeling and quality control, and documents all steps in the laboratory information management system (LIMS). This role requires strong technical skills, adherence to CAP/CLIA/GLP regulatory standards, excellent attention to detail, and clear communication with pathologists, lab leadership, and multidisciplinary teams.

Core Responsibilities

Primary Functions

  • Accession, label, and log incoming surgical, biopsy, and research tissue specimens into the LIMS with complete, accurate patient and specimen identifiers, ensuring chain-of-custody and specimen integrity for diagnostic workflows.
  • Prepare and fix tissues using appropriate fixatives and fixation protocols; apply decalcification procedures when required; document fixation times to maintain reproducible histologic outcomes.
  • Operate automated and manual tissue processors to dehydrate, clear, and infiltrate specimens with paraffin per validated SOPs; monitor runs and troubleshoot processing issues to prevent specimen loss.
  • Embed processed tissue in paraffin blocks with consistent orientation and facing technique to optimize diagnostic sections for pathologist review; maintain block inventory and storage procedures.
  • Perform microtomy to produce high-quality paraffin sections at specified thicknesses, handling delicate tissues to prevent compression, tears, or artifacts; maintain sharp blades and microtome maintenance logs.
  • Prepare and cut frozen sections (cryotomy) for intraoperative consultations (STAT/frozen cases), ensuring rapid turnaround, high-quality sections, and accurate communication of results to pathology and surgical teams.
  • Stain routine hematoxylin & eosin (H&E) slides with precision and consistency; perform and quality-check special stains (e.g., PAS, trichrome, reticulin) following validated protocols and reagent control criteria.
  • Conduct immunohistochemistry (IHC) assays, including reagent preparation, antigen retrieval, antibody incubation, detection systems, and positive/negative control interpretation; participate in assay validation and optimization.
  • Operate and maintain automated stainers, coverslippers, and slide dryers; perform routine preventative maintenance, document service activities, and escalate equipment failures to biomedical engineering when necessary.
  • Inspect stained slides for technical quality, completeness, and labeling accuracy; complete internal quality control checks and release slides for pathologist interpretation in a timely manner.
  • Accurately label and mount slides, apply coverslips or automated coverslippers, and prepare final slide sets for pathologist sign-out; ensure slide sets are complete and correctly organized.
  • Scan glass slides using digital pathology scanners where available; QC scanned images for artifacts and collaborate with digital pathology teams to ensure image integrity for telepathology and AI analysis.
  • Manage reagents, buffers, and antibody inventories: prepare working reagents, track lot numbers and expiration dates, and perform reagent qualification and stability checks per regulatory requirements.
  • Maintain comprehensive laboratory documentation, including batch records, instrument logs, maintenance reports, QA/QC logs, and LIMS entries to support traceability and regulatory audits (CAP/CLIA).
  • Participate in method validation, verification, and troubleshooting of new stains, IHC assays, and instrumentation; document validation data and SOP updates to expand laboratory testing capabilities.
  • Follow laboratory safety protocols for handling biohazardous tissues, chemical reagents (formalin, xylene, alcohol), and sharps; manage hazardous waste segregation and disposal in compliance with institutional and regulatory guidelines.
  • Respond to STAT and priority specimen requests, triaging workload and reprioritizing tasks while maintaining documentation of turnaround time (TAT) to meet clinical service level agreements.
  • Provide technical training and mentorship to histology technicians, students, and new staff; create and update training materials, competency assessments, and skill checklists.
  • Collaborate with pathologists, laboratory managers, and clinicians to clarify specimen adequacy, perform special requests, and support research protocols or clinical trials that require specialized processing or staining.
  • Participate in quality improvement initiatives, root cause analyses for discrepant specimens or staining failures, and corrective action plans to enhance assay reliability and laboratory throughput.
  • Adhere to and help develop SOPs, safety protocols, and laboratory policies; actively contribute to CAP/CLIA inspections, proficiency testing, and continuous competency assessment programs.
  • Maintain a clean, organized workspace; ensure slide storage, paraffin block filing, and specimen archives are indexed, retrievable, and protected from damage or environmental extremes.
  • Support histology billing and documentation by tagging special procedures and capturing appropriate procedure codes in LIMS for accurate reimbursement and internal metrics.
  • Engage in cross-functional projects such as digital pathology adoption, assay automation, or implementation of new LIMS modules; provide technical input and test plans for seamless integration.

Secondary Functions

  • Assist in research and validation studies by preparing protocol-specific tissue processing, staining, and documentation to meet investigative endpoints.
  • Help coordinate interdepartmental specimen transport, courier schedules, and sample prioritization for time-sensitive cases.
  • Provide input to purchasing and vendor evaluation for histology instrumentation, reagents, and consumables to improve cost-efficiency and technical performance.
  • Participate in departmental meetings and contribute data for workload metrics, TAT improvement plans, and staffing models.
  • Support external quality assurance programs and proficiency testing by preparing and submitting required comparators and documentation.
  • Contribute to development and maintenance of digital slide libraries and educational slide sets for training and pathology education.
  • Provide coverage across shifts for weekend or on-call frozen section services when required, ensuring continuous clinical support.
  • Assist the lab safety officer with periodic audits, chemical inventories, and biological safety updates to maintain a compliant lab environment.

Required Skills & Competencies

Hard Skills (Technical)

  • Proficient in tissue accessioning and LIMS data entry with accurate specimen tracking and label generation.

Education & Experience

Educational Background

Minimum Education:

  • Associate degree in Histotechnology, Medical Laboratory Technology, or related biological science, OR completion of an accredited histotechnology program.

Preferred Education:

  • Bachelor's degree in Biology, Biomedical Science, Medical Laboratory Science, or equivalent plus ASCP HT or HTL certification.
  • Advanced coursework or certificate in immunohistochemistry, molecular pathology workflows, or digital pathology.

Experience Requirements

Typical Experience Range:

  • ASCP HT or HTL certification or equivalent national credential preferred. Experience with CAP/CLIA-regulated labs and LIMS is highly desirable.
