Core Responsibilities

Primary Functions

  • Provide direct and compassionate personal care, including assistance with bathing, grooming, dressing, and toileting, always prioritizing client dignity and comfort.
  • Assist clients with mobility and transfers, utilizing proper body mechanics and safety devices like gait belts or Hoyer lifts to prevent injury.
  • Support clients with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) as outlined in their personalized care plan.
  • Prepare and serve nutritious meals and snacks, adhering to specific dietary restrictions, allergies, and client preferences.
  • Administer medication reminders to ensure clients take their prescribed medications on schedule as directed by their healthcare provider.
  • Monitor and record vital signs, such as blood pressure, temperature, and pulse, reporting any significant changes or concerns to the nursing supervisor immediately.
  • Engage clients in meaningful conversation, companionship, and mentally stimulating activities like reading, playing games, or sharing hobbies to combat loneliness.
  • Accompany clients to medical appointments, errands, and social outings, providing transportation and assistance as needed.
  • Observe and report any changes in the client's physical condition, mental state, or behavior to the care management team.
  • Perform light housekeeping duties essential to a safe and clean environment, including laundry, washing dishes, and tidying living areas.
  • Follow all health and safety protocols, including infection control procedures, to ensure a safe environment for both the client and yourself.
  • Provide specialized care for clients with chronic conditions such as dementia, Alzheimer's, or Parkinson's, using patience and validated techniques.
  • Assist with prescribed range-of-motion exercises and other therapeutic activities to help clients maintain their physical function.
  • Offer emotional support and encouragement to clients and their families, building a trusting and positive relationship.
  • Ensure client safety at all times, identifying and mitigating potential fall risks and other hazards within the home environment.

Secondary Functions

  • Maintain detailed, accurate, and timely documentation of services provided, client activities, and any observed changes in daily care logs or electronic charting systems.
  • Communicate effectively and professionally with the client's family, providing updates on their well-being and collaborating on the care plan.
  • Collaborate with the broader healthcare team, including nurses, therapists, and case managers, to ensure a coordinated approach to client care.
  • Manage household needs, such as grocery shopping, picking up prescriptions, and organizing mail.
  • Provide respite care for family caregivers, offering them temporary relief from their caregiving responsibilities.

Education & Experience

Educational Background

Minimum Education:

  • High School Diploma or GED equivalent.
  • Must be able to pass a state and federal background check.
