Core Responsibilities
Primary Functions
- Provide direct and compassionate personal care, including assistance with bathing, grooming, dressing, and toileting, always prioritizing client dignity and comfort.
- Assist clients with mobility and transfers, utilizing proper body mechanics and safety devices like gait belts or Hoyer lifts to prevent injury.
- Support clients with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) as outlined in their personalized care plan.
- Prepare and serve nutritious meals and snacks, adhering to specific dietary restrictions, allergies, and client preferences.
- Administer medication reminders to ensure clients take their prescribed medications on schedule as directed by their healthcare provider.
- Monitor and record vital signs, such as blood pressure, temperature, and pulse, reporting any significant changes or concerns to the nursing supervisor immediately.
- Engage clients in meaningful conversation, companionship, and mentally stimulating activities like reading, playing games, or sharing hobbies to combat loneliness.
- Accompany clients to medical appointments, errands, and social outings, providing transportation and assistance as needed.
- Observe and report any changes in the client's physical condition, mental state, or behavior to the care management team.
- Perform light housekeeping duties essential to a safe and clean environment, including laundry, washing dishes, and tidying living areas.
- Follow all health and safety protocols, including infection control procedures, to ensure a safe environment for both the client and yourself.
- Provide specialized care for clients with chronic conditions such as dementia, Alzheimer's, or Parkinson's, using patience and validated techniques.
- Assist with prescribed range-of-motion exercises and other therapeutic activities to help clients maintain their physical function.
- Offer emotional support and encouragement to clients and their families, building a trusting and positive relationship.
- Ensure client safety at all times, identifying and mitigating potential fall risks and other hazards within the home environment.

Minimum Education:
