Core Responsibilities

Primary Functions

  • Provide hands-on assistance with personal care routines, including bathing, grooming, dressing, and oral hygiene, while promoting client dignity and independence.
  • Support client mobility by assisting with transfers from bed to chair, walking, and the use of assistive devices like walkers, canes, or wheelchairs.
  • Ensure proper nutrition and hydration by planning, preparing, and serving meals and snacks according to the client's specific dietary plan and preferences.
  • Administer medication reminders to ensure clients take their prescribed medications on time and as directed by their healthcare provider.
  • Monitor and record vital signs, such as blood pressure, temperature, pulse, and respiration rate, reporting any significant changes or concerns to the nursing supervisor.
  • Assist clients with toileting and incontinence care in a respectful and discreet manner, maintaining a high standard of hygiene.
  • Perform light housekeeping duties essential for a safe and healthy environment, including doing dishes, laundry, changing bed linens, and tidying client living areas.
  • Provide compassionate companionship and emotional support, engaging clients in conversation, reading, games, and other social activities to combat loneliness and promote mental well-being.
  • Accompany and transport clients to medical appointments, errands like grocery shopping or pharmacy pick-ups, and social outings as specified in the care plan.
