Core Responsibilities
Primary Functions
- Provide hands-on personal care, including bathing, dressing, grooming, and toileting, with a steadfast focus on preserving client dignity and promoting self-reliance.
- Assist clients with safe mobility and transfers, utilizing proper body mechanics and assistive devices like Hoyer lifts or gait belts to ensure the safety of both client and caregiver.

- Administer or provide timely reminders for medications as explicitly prescribed by the physician and meticulously document each administration in the client's health record.
- Plan and prepare nutritious, appealing meals and snacks that adhere to the client's specific dietary needs, restrictions, and personal preferences, assisting with feeding when necessary.
- Perform essential light housekeeping duties that contribute to a safe, clean, and healthy living environment, including laundry, dishwashing, changing bed linens, and tidying client living areas.
- Offer compassionate companionship and crucial emotional support by engaging clients in conversation, reading aloud, playing games, and participating in other recreational activities to promote mental and social well-being.
- Accompany clients to medical appointments, social engagements, and other errands, often providing transportation and physical assistance to facilitate their participation in community life.
- Observe, document, and report any changes in the client’s physical condition, mental state, or behavior to the healthcare team in a clear, objective, and timely manner.

- Adhere strictly to all infection control protocols, safety guidelines, and emergency procedures, including the consistent use of universal precautions to protect clients and yourself.
