Core Responsibilities

Primary Functions

  • Conducts comprehensive initial and ongoing patient assessments, including the OASIS (Outcome and Assessment Information Set), to accurately evaluate the patient's physical, psychosocial, and environmental status.
  • Develops, implements, and regularly evaluates individualized patient care plans in collaboration with the physician, patient, and family to address specific needs and achieve defined goals.
  • Provides expert, hands-on skilled nursing care, including complex wound care, dressing changes, and VAC therapy management, following established protocols and physician orders.
  • Administers medications and treatments as prescribed, including intravenous (IV) therapy, infusions, and injections, while meticulously monitoring for therapeutic effects and adverse reactions.
  • Performs in-depth patient and caregiver education on disease processes, medication management, treatment plans, and self-care techniques to promote patient independence and empowerment.
  • Executes physician's orders precisely and coordinates any necessary changes to the plan of care, ensuring timely communication with the ordering provider.
  • Acts as the primary case manager for an assigned caseload of patients, coordinating services with other disciplines such as physical therapy, occupational therapy, and social work.
  • Completes and submits all clinical documentation accurately and within agency timeframes, utilizing EMR systems like Homecare Homebase or Epic to reflect the patient's condition and the care provided.
  • Performs skilled procedures such as catheter insertion and maintenance, ostomy care, tracheostomy care, and blood glucose monitoring.
  • Conducts medication reconciliation during key points of care transition to ensure patient safety and prevent medication discrepancies.
  • Monitors patients for changes in clinical status, promptly reporting significant findings, new symptoms, or concerns to the patient's physician and the clinical supervisor.
  • Supervises and delegates tasks to Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs), providing guidance and ensuring the quality of care delivered.
  • Facilitates effective communication between the patient, family, physicians, and the entire interdisciplinary care team to ensure a holistic and coordinated approach to care.
  • Assesses the home environment for safety risks and recommends modifications or equipment to create a safer setting for the patient.
  • Manages and provides care for patients with a wide range of chronic conditions, such as Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), and Diabetes.
  • Obtains necessary physician orders for all treatments, medications, and changes in the frequency or duration of care.
  • Prepares patients and their families for discharge from home health services, ensuring they have the knowledge and resources for continued self-management.
  • Participates in the agency's on-call rotation as required, providing telephonic triage and making urgent visits to address emergent patient needs after hours.
  • Draws blood specimens for diagnostic testing and delivers them to the appropriate laboratory, following all safety and handling protocols.
  • Continuously updates professional knowledge and skills by participating in continuing education opportunities, in-service training, and professional organizations.
  • Assesses and manages patient pain levels using appropriate pharmacological and non-pharmacological interventions, documenting effectiveness and adjusting the plan as needed.

  • Participate in quality assurance and performance improvement (QAPI) activities, including chart audits, data collection, and process review, to enhance patient outcomes.
  • Contribute to the development and refinement of clinical pathways, patient education materials, and agency-wide care protocols.
  • Collaborate effectively with intake coordinators, schedulers, and billing departments to ensure seamless care transitions and accurate documentation for reimbursement.
  • Actively participate in interdisciplinary team meetings, case conferences, and mandatory staff meetings to review patient progress and collaboratively adjust plans of care.

Education & Experience

Educational Background

Minimum Education:

Associate's Degree in Nursing (ADN) from an accredited institution. Current, unencumbered Registered Nurse (RN) license in the state of practice.

Preferred Education:

Bachelor of Science in Nursing (BSN).

Experience Requirements

Typical Experience Range: 1-3 years of professional nursing experience.

Preferred: A minimum of one year of direct patient care experience in a medical-surgical, acute care, critical care, or skilled nursing facility setting is strongly preferred. Previous experience in a home health or hospice environment is a significant advantage.
