Core Responsibilities
Primary Functions
- Conduct comprehensive initial and ongoing nursing assessments of patients, considering their physical, psychosocial, and spiritual needs to establish a holistic baseline.
- Develop, implement, and continuously evaluate an individualized patient Plan of Care in collaboration with the patient, family, and interdisciplinary group (IDG).
- Expertly manage patient symptoms, including complex pain, nausea, and dyspnea, by utilizing advanced clinical skills and titrating medications in coordination with the physician.
- Serve as the primary point of contact and coordinator for the interdisciplinary team, facilitating seamless communication and ensuring all members are aligned with the Plan of Care.
- Provide comprehensive education to patients and their families regarding the disease process, prognosis, symptom management, and the emotional aspects of end-of-life care.
- Deliver skilled, hands-on nursing care as required, which may include wound care, medication administration, and management of medical equipment in the patient's home environment.
- Supervise and delegate tasks to Licensed Practical/Vocational Nurses (LPN/LVNs) and Certified Hospice Aides, ensuring they provide care in accordance with the established plan.
- Maintain meticulous, accurate, and timely clinical documentation in the electronic medical record (EMR) system, reflecting the patient's condition and the care provided.
- Actively participate in regular interdisciplinary group meetings, presenting patient cases, contributing clinical insights, and collaborating on care plan modifications.
- Communicate effectively and proactively with the patient's attending physician and the hospice medical director to report changes in condition and obtain necessary orders.
- Ensure all nursing practices and documentation adhere to federal, state, and local regulations, as well as the organization's policies and Medicare Conditions of Participation.
- Assess the patient's home environment for safety and identify any needs for durable medical equipment (DME) or supplies, coordinating their delivery and setup.
- Manage the patient's medication profile, reconciling medications at each visit, and ordering refills and new prescriptions to ensure continuity of care.
- Perform comprehensive admission procedures for new patients entering the hospice program, establishing the initial Plan of Care and orienting the family to services.
- Facilitate the process for patient recertification, providing the necessary clinical documentation to support continued eligibility for the hospice benefit.
- Provide direct emotional support to patients and families, offering a compassionate presence and connecting them with social work and spiritual care resources as needed.
- Manage a designated caseload of patients, prioritizing visits and tasks effectively to meet the dynamic needs of each individual and family.
- Actively participate in the on-call rotation schedule, providing telephonic triage and making urgent visits to address emergent patient needs after hours.

Education & Experience
Educational Background
Minimum Education:
Associate's Degree in Nursing (ADN) from an accredited institution. Must hold a current, active, and unencumbered Registered Nurse (RN) license in the state of practice.
