Core Responsibilities
Primary Functions
- Conduct comprehensive initial and ongoing assessments of hospice patients' physical, emotional, psychosocial, and spiritual needs to establish a holistic understanding and baseline.
- Develop, implement, and continuously evaluate individualized patient care plans in collaboration with the patient, family, and the full interdisciplinary group (IDG).
- Provide expert clinical care, focusing on state-of-the-art pain and symptom management to ensure patient comfort and quality of life.
- Act as the primary case manager, expertly coordinating all patient/family services and personnel, including RNs, LPNs, aides, social workers, spiritual counselors, and volunteers.
- Educate patients and their families on the disease process, progression, and expectations for end-of-life, empowering them to make informed decisions.
- Perform skilled nursing interventions, including wound care, medication administration, and management of medical equipment, in various settings such as private homes and long-term care facilities.
- Offer direct emotional, psychological, and spiritual support to patients and their loved ones, providing a consistent and reassuring presence.
- Facilitate difficult conversations regarding goals of care, advance directives, and end-of-life wishes with profound empathy and professionalism.
- Maintain a manageable caseload, ensuring each patient and family receives the dedicated time and attention required for high-quality hospice care.
- Respond to and manage after-hours calls, emergent patient needs, and crisis situations with calm, decisive, and compassionate action.
- Assess the home environment for safety and determine the need for durable medical equipment (DME) and other adaptive resources.
- Provide bereavement support and grief counseling resources to families and caregivers following the death of the patient, ensuring continuity of care.
- Act as a steadfast patient advocate, ensuring their wishes are honored and they receive the full scope of benefits available under hospice care.
- Supervise and support Hospice Aides (HHAs/CNAs) in the field, ensuring the plan of care is followed and providing ongoing education.
- Pronounce patient deaths in accordance with state regulations and agency policy, providing immediate support to the bereaved family.
Secondary Functions
- Meticulously and promptly document all patient interactions, clinical assessments, interventions, and care plan updates in the Electronic Medical Record (EMR) system.
- Actively participate in weekly Interdisciplinary Group (IDG) meetings to review patient cases, update care plans, and collaborate on complex care challenges.
- Engage in community outreach and educational initiatives to promote understanding of hospice and palliative care services among healthcare partners and the public.
- Assist in the orientation, training, and mentorship of new clinical staff, sharing best practices and fostering a supportive team environment.
- Participate in the organization's quality assurance and performance improvement (QAPI) programs to identify areas for clinical excellence and enhanced patient outcomes.
- Maintain up-to-date knowledge of local, state, and federal hospice regulations, including Medicare Conditions of Participation, to ensure full compliance.
- Support ad-hoc data requests and exploratory data analysis for quality reporting.
- Contribute to the organization's data strategy and roadmap.
- Collaborate with business units to translate data needs into engineering requirements.
- Participate in sprint planning and agile ceremonies within the data engineering team.
Required Skills & Competencies

Education & Experience
Educational Background
Minimum Education:
- Associate's Degree in Nursing (ASN) with relevant experience, or a Master's Degree in Social Work (MSW) from an accredited program.
Preferred Education:
- Bachelor of Science in Nursing (BSN).
- Certification as a Certified Hospice and Palliative Nurse (CHPN) or Advanced Certified Hospice and Palliative Social Worker (ACHP-SW).

Experience Requirements
Typical Experience Range: 2-5 years of clinical experience in a healthcare setting.
