Core Responsibilities

Primary Functions

  • Assist patients with Activities of Daily Living (ADLs), including bathing, grooming, dressing, and toileting, while promoting patient independence and preserving dignity.
  • Accurately measure and record vital signs, such as temperature, blood pressure, pulse, and respiration rates, and report any significant deviations or abnormalities to the supervising nurse immediately.
  • Provide direct assistance with patient mobility, including safe patient transfers, turning, positioning, and ambulation, utilizing proper body mechanics and lifting equipment to prevent injury.
  • Promptly respond to patient call lights and inquiries, anticipating patient needs to create a responsive and supportive environment.
  • Document all patient care activities, observations, and vital signs in the Electronic Medical Record (EMR) system in a timely and accurate manner.
  • Assist nursing and medical staff with a variety of bedside procedures, gathering necessary supplies and providing support to the patient.
  • Actively observe patients for any changes in physical condition, behavior, or mental status, and communicate these critical observations to the nursing team for further assessment.
  • Collect various clinical specimens, such as urine, stool, and sputum, using proper collection techniques and ensuring they are labeled correctly for laboratory analysis.
  • Provide compassionate emotional support and companionship to patients and their families, fostering a therapeutic and trusting relationship.
  • Perform simple, non-sterile dressing changes and other delegated nursing tasks as directed and supervised by a Registered Nurse.
  • Assist with patient nutritional needs by distributing meal trays, setting up patients for meals, feeding patients when necessary, and accurately documenting food and fluid intake/output.
  • Reinforce patient and family education on topics such as mobility protocols, safety measures, and unit procedures as initiated by the Registered Nurse.

Secondary Functions

  • Maintain a clean, safe, and organized patient environment by tidying rooms, changing bed linens, and removing clutter to ensure infection control standards are met.
  • Proactively stock patient rooms and nursing stations with essential medical supplies, linens, and personal protective equipment (PPE).
  • Facilitate the smooth admission, transfer, and discharge of patients by preparing rooms, transporting belongings, and providing orientation to the unit.
  • Safely transport patients via wheelchair or stretcher to and from various departments for diagnostic tests, procedures, or therapies.
  • Perform post-mortem care with the utmost respect and dignity, following hospital protocol and providing support to the family.
  • Manage and maintain the cleanliness and operational readiness of unit-specific medical equipment, reporting any malfunctions for repair.
  • Assist with basic clerical duties at the nursing station, such as answering phones, relaying messages, and organizing patient charts when needed.
  • Actively participate in patient safety initiatives, including implementing fall precautions, utilizing bed alarms correctly, and participating in safety huddles.

Required Skills & Competencies

Hard Skills (Technical)

  • Vital Signs Monitoring: Proficiency in accurately taking and recording temperature, pulse, respiration, and blood pressure.
  • BLS/CPR Certification: Current Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification is mandatory.
  • EMR Charting: Experience with Electronic Medical Record (EMR) systems like Epic or Cerner for documentation.
  • Patient Handling & Mobility: Knowledge of safe patient transfer techniques, including the use of lifts and other mobility aids.
  • Infection Control: Strong understanding and application of infection control principles and use of Personal Protective Equipment (PPE).
  • Basic Medical Terminology: Solid grasp of common medical terms and abbreviations used in a clinical setting.
  • Specimen Collection: Competency in collecting clinical specimens according to established protocols.

Education & Experience

Educational Background

Minimum Education:

  • High School Diploma or GED equivalent.
  • Completion of a Certified Nursing Assistant (CNA) program or equivalent training.
  • Current BLS (Basic Life Support) certification from the American Heart Association.

Preferred Education:

  • Associate's Degree in a health-related field.
  • Enrollment in a nursing (LPN or RN) program.

Experience Requirements

Typical Experience Range: 0 - 3 years.

Preferred:

  • At least 1 year of hands-on experience as a CNA, PCT, or similar role within a hospital, long-term care, or acute care facility.
  • Experience working in a specific specialty (e.g., Med-Surg, ICU, Emergency Department) is a significant plus.
