Core Responsibilities

Primary Functions

  • Conduct comprehensive clinical assessments including biopsychosocial, behavioral, functional, and risk assessments to identify strengths, needs, triggers, protective factors, and safety concerns for individuals across the lifespan.
  • Develop individualized, evidence-based intervention plans that align with client goals, measurable outcomes, and best-practice modalities (CBT, MI, DBT, TF-CBT, Positive Behavior Support, Wraparound), ensuring plans are culturally responsive and trauma-informed.
  • Provide one-on-one therapeutic interventions and coaching to clients using evidence-based techniques (motivational interviewing, cognitive behavioral strategies, behavioral activation, skill-building) to reduce symptoms and increase functioning.
  • Lead crisis intervention and de-escalation efforts both in person and via telehealth, create safety plans, coordinate emergency responses, and liaise with emergency services, hospitals, and guardians as necessary to manage acute risk.
  • Facilitate multi-disciplinary care planning meetings, care coordination, and interagency communication to ensure continuity of care between psychiatry, primary care, schools, social services, and community providers.
  • Create, implement, and monitor behavior support plans and positive behavior interventions for clients with complex behavioral health needs, adjusting interventions based on ongoing data and functional behavior assessments.
  • Provide family systems-based interventions, caregiver training, and psychoeducation to increase family capacity, promote positive parenting strategies, and support transitions between levels of care (home, school, inpatient, community).
  • Deliver group-based intervention sessions, workshops, and psychoeducational curricula (coping skills, anger management, social skills, relapse prevention) tailored to population needs and program goals.
  • Perform ongoing progress monitoring and outcomes measurement using validated tools and metrics; analyze client-level and program-level data to recommend adjustments and demonstrate efficacy.
  • Document clinical contacts, progress notes, safety assessments, care plans, and discharge summaries in electronic medical records (EMR) accurately and in compliance with organizational, state, and federal regulations (HIPAA).
  • Design and deliver staff training, clinical coaching, and competency development for frontline practitioners, therapists, behavioral technicians, and school staff to build system-wide capacity for evidence-based interventions.
  • Conduct implementation planning and fidelity monitoring for new programs or interventions, including pilot designs, training plans, fidelity checklists, and corrective action strategies to maintain treatment integrity.
  • Perform referrals and resource navigation for clients, connecting them with community supports, housing resources, vocational services, substance use treatment, and specialty behavioral health services.
  • Lead program-level quality improvement initiatives, develop key performance indicators (KPIs), prepare performance reports, and present findings to leadership with recommendations for practice improvements.
  • Support discharge planning and transition management, ensuring continuity of care, follow-up appointments, community supports, and relapse prevention strategies are in place.
  • Collaborate with schools, juvenile justice, child welfare, primary care, and specialty providers to co-create individualized education or service plans and provide consultation on behavior management and accommodations.
  • Conduct case reviews and utilization management consultations to ensure appropriateness of care, length of service, and alignment with clinical goals and payer requirements.
  • Manage caseloads with prioritization of high-risk individuals, perform regular case staffing, and escalate clinical or ethical concerns appropriately to supervisors and clinical directors.
  • Support program development by contributing to service design, clinical policies, standard operating procedures, and documentation templates that support scalability and compliance.
  • Participate in multidisciplinary research, evaluation projects, and grant-funded initiatives to test, refine, and scale evidence-based intervention models and to publish or present outcomes internally and externally.
  • Maintain professional licensure, credentialing requirements, and relevant continuing education; serve as a subject matter expert on current evidence-based practices and regulatory requirements.

Education & Experience

Educational Background

Minimum Education:

  • Bachelor’s degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing, Special Education, Public Health, or related human services field; plus relevant clinical experience.

Preferred Education:

  • Master’s degree (MSW, LPC, LMFT, MEd, Clinical Psychology) or equivalent graduate clinical training.
  • Active licensure (LCSW, LPC, LMFT, LP, RN) preferred for clinical supervisory responsibilities and billing.

Experience Requirements
