Core Responsibilities

Primary Functions

  • Serve as the primary clinical liaison, facilitating clear and consistent communication between patients, families, physicians, and multidisciplinary healthcare teams to ensure a unified approach to care.
  • Conduct comprehensive, in-depth clinical assessments of patients' physical, psychosocial, and environmental health to accurately determine their needs and establish an appropriate level of care.
  • Develop, implement, and dynamically evaluate individualized patient care plans in close collaboration with the interdisciplinary team, making adjustments as needed to achieve optimal health outcomes.
  • Orchestrate the seamless transition of patients across different levels of care and healthcare settings, such as from hospital to home health or a skilled nursing facility, minimizing gaps in service.
  • Act as a dedicated patient advocate, ensuring their voice is heard, their rights are protected, and they receive the necessary resources and information to make informed decisions about their health.
  • Provide expert clinical consultation and evidence-based education to patients, families, and facility staff on disease processes, treatment options, and preventative health strategies.
  • Meticulously review patient charts, medical records, and clinical documentation to ensure accuracy, completeness, and full compliance with regulatory standards and internal policies.
  • Lead case conferences and interdisciplinary team meetings, presenting patient cases, facilitating discussion, and driving consensus on care plan strategies and goals.
  • Monitor, track, and analyze patient outcomes and quality metrics to identify trends, pinpoint areas for improvement, and implement quality assurance initiatives.
  • Function as a subject matter expert on clinical best practices, healthcare regulations (including HIPAA and JCAHO), and insurance-related requirements (e.g., Medicare, Medicaid).
  • Investigate and respond to clinical inquiries, concerns, or complaints from patients, families, or staff, employing strong problem-solving skills to reach a satisfactory resolution.
  • Coordinate the procurement of necessary medical equipment, specialty services, and community resources to fully support the patient's care plan and recovery process.
  • Mentor and provide clinical guidance to junior nursing staff, LPNs, and medical assistants, fostering a culture of continuous learning and professional development.
  • Participate in the development and revision of clinical policies, procedures, and protocols to ensure they reflect the latest evidence-based practices and regulatory changes.
