Core Responsibilities
Primary Functions
- Patient Care Coordination: Strategically orchestrate and oversee the entire patient care continuum, ensuring seamless transitions between different levels of care, from initial assessment and admission to treatment, discharge, and post-discharge follow-up.
- Interdisciplinary Collaboration: Serve as the primary communication link for the multidisciplinary team, including physicians, specialists, therapists, social workers, and nursing staff, by facilitating daily rounds, case conferences, and care planning meetings.

- Clinical Resource & Mentorship: Function as a clinical expert and resource for staff nurses, providing guidance, mentorship, and education on complex patient cases, clinical protocols, and best practices.
- Discharge Planning: Proactively manage and coordinate all aspects of discharge planning, including arranging for home health services, durable medical equipment (DME), skilled nursing facility placement, and patient/family education to ensure a safe transition home.

- Patient Education: Provide comprehensive education to patients and families regarding their diagnosis, treatment plan, medications, and self-management strategies to empower them in their own care.
- Regulatory Compliance: Ensure all care coordination activities are documented accurately and adhere to internal policies, as well as state, federal, and JCAHO/DNV regulations.
