Primary Functions

- Provide evidence-based chronic disease management programs—including medication management, lifestyle modification counseling, and structured follow-up—to improve disease control and patient outcomes (A1c, BP, lipid targets).
- Deliver patient and family education on diagnosis, therapeutic options, medication regimens, preventive care, and self-management strategies tailored to health literacy and cultural needs.
