Core Responsibilities

Primary Functions

  • Develop, implement, and critically evaluate individualized care plans for patients with long-term conditions such as diabetes, asthma, COPD, and hypertension.
  • Conduct comprehensive patient health assessments, including taking detailed medical histories and performing physical examinations to diagnose and treat common health issues.
  • Administer a full range of adult and childhood immunizations and vaccinations according to national schedules, patient group directions (PGDs), and individual health needs.

  • Provide expert, evidence-based health promotion and disease prevention advice on critical topics like smoking cessation, weight management, nutrition, and physical activity.
  • Independently manage and run nurse-led clinics for chronic disease management, ensuring patients receive timely reviews, proactive care, and achieve clinical targets.
  • Undertake cervical screening (smear tests) and provide sensitive, patient-centered counseling on sexual health, family planning, and contraception options.
  • Perform a variety of diagnostic procedures including venepuncture, electrocardiograms (ECGs), and spirometry, and provide an initial interpretation of the results.
  • Triage patient calls and walk-in appointments, applying robust clinical judgment to accurately assess urgency and navigate patients to the most appropriate care pathway.
  • Maintain meticulous, accurate, and contemporaneous patient records using electronic health record systems (e.g., EMIS Web, SystmOne), ensuring data integrity and patient confidentiality.
  • Assist General Practitioners (GPs) with minor surgical procedures, ensuring a sterile field, preparing patients, and providing pre- and post-operative care instructions.
  • Manage the practice's recall systems for patients requiring follow-up appointments for chronic conditions, screening programs, and scheduled vaccinations.
  • Provide comprehensive travel health consultations, assessing risk, administering necessary vaccinations, and offering preventative advice for international travelers.
  • Act as a central resource for patients and their families, delivering education and support to empower them in the self-management of their health conditions.
  • Participate actively in clinical audits, quality improvement initiatives, and research projects to enhance the standard of care and patient outcomes within the practice.
  • Respond effectively to medical emergencies within the practice, providing immediate life support and stabilizing patients until emergency services arrive.
  • Manage the cold chain for all vaccines and temperature-sensitive medications, ensuring proper storage, handling, and documentation to maintain efficacy and patient safety.
  • Mentor and provide clinical support to junior nursing staff, healthcare assistants, and student nurses, fostering a positive and collaborative learning environment.
  • Conduct ear assessments and perform aural care procedures, including ear syringing/irrigation, for patients presenting with blockages or related symptoms.
  • Liaise effectively with the wider multidisciplinary team, including GPs, pharmacists, mental health services, and secondary care specialists, to ensure seamless and integrated patient care.
  • Stay current with evolving clinical guidelines, best practices, and mandatory training requirements, taking full responsibility for your continuing professional development (CPD).
  • Prescribe medications and treatments as an independent or supplementary prescriber, or work within established Patient Group Directions (PGDs).

Secondary Functions

  • Assist in maintaining and calibrating clinical equipment, ensuring it is clean, functional, and ready for use.
  • Manage the inventory of clinical supplies, dressings, and vaccines, overseeing stock levels and placing orders to prevent shortages.
  • Contribute to the development, review, and implementation of clinical policies, protocols, and procedures within the practice.
  • Participate actively in regular practice meetings, clinical huddles, and team-building activities to improve operational efficiency.

Education & Experience

Educational Background

Minimum Education:
