Core Responsibilities
Primary Functions
- Conduct comprehensive initial and ongoing assessments of clients in their homes to determine specific care needs and develop individualized, patient-centered plans of care.
- Directly supervise, orient, and evaluate the performance of a team of field clinicians, including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Home Health Aides (HHAs).
- Develop, implement, and continuously review client-specific plans of care in collaboration with the patient, their family, and their physician to ensure optimal outcomes.
- Perform routine and ad-hoc supervisory visits to client homes to assess the quality of care being delivered, ensure adherence to the care plan, and provide on-the-spot coaching to field staff.
- Act as the primary clinical point of contact for clients, families, and healthcare providers, facilitating clear and consistent communication regarding patient status, concerns, and progress.
- Ensure all clinical documentation is accurate, complete, and submitted in a timely manner, conducting regular chart audits to maintain compliance with company policies and regulatory standards.
- Manage and coordinate field staff schedules to ensure all client shifts are covered with appropriately skilled clinicians, addressing call-outs and staffing emergencies as they arise.
- Provide direct, hands-on skilled nursing care to clients as needed, including during staff shortages or for high-acuity cases requiring advanced clinical skills.
- Investigate and resolve client or family complaints and grievances in a professional and timely manner, implementing corrective actions to prevent recurrence.
- Lead the onboarding, training, and competency validation for all new clinical field staff, ensuring they are fully prepared to meet client needs.
- Enforce and provide education on infection control protocols, safety procedures, and emergency preparedness plans for both staff and clients.
- Participate in a rotating on-call schedule to provide 24/7 clinical support and guidance for field staff, clients, and their families.
- Facilitate regular team meetings to disseminate information, provide clinical education, and foster a collaborative and supportive team environment.
- Conduct annual performance reviews for direct reports, setting goals and identifying opportunities for professional development and growth.
- Monitor and manage the inventory of nursing supplies and medical equipment in client homes, coordinating with vendors for ordering and maintenance.
Secondary Functions

Required Skills & Competencies
Hard Skills (Technical)
- Active State RN License: Must possess a current, unencumbered Registered Nurse license in the state of operation.
- Clinical Assessment Expertise: Demonstrated proficiency in performing comprehensive head-to-toe physical, psychosocial, and environmental assessments.
- EMR/EHR Proficiency: Strong computer skills and experience working with Electronic Medical Record systems for charting, care planning, and scheduling.
- Regulatory Knowledge: In-depth understanding of state and federal home care regulations, as well as accreditation standards (e.g., CHAP, JCAHO).

Typical Experience Range: 3-5 years
Preferred:
A minimum of 3 years of direct clinical experience as a Registered Nurse, with a strong preference for a background in home health, private duty, hospice, or critical care (ICU/ER). At least 1-2 years of experience in a formal or informal leadership role (e.g., Clinical Supervisor, Charge Nurse, Team Lead, or Preceptor) is highly desired.
