An Upper GI Surgeon (Foregut Surgeon) is a board-certified/eligible surgeon specializing in the diagnosis, surgical management, and longitudinal care of patients with diseases of the esophagus, stomach, gastroesophageal junction and proximal small bowel. Core Responsibilities

Primary Functions

  • Perform comprehensive pre-operative evaluations and informed consent discussions for patients with benign and malignant upper gastrointestinal conditions, incorporating oncologic principles, staging data, nutritional status, and expected functional outcomes.
  • Plan and execute complex elective and emergency upper GI surgeries including Ivor Lewis and McKeown esophagectomy, subtotal and total gastrectomy, transhiatal esophagectomy, anti-reflux procedures (fundoplication, LINX), paraesophageal hernia repair, Heller myotomy, and foregut reconstructions with meticulous attention to oncologic margins and lymphadenectomy when indicated.
  • Deliver minimally invasive and robotic-assisted foregut procedures (laparoscopic, thoracoscopic, and robotic esophagectomy/gastrectomy, robotic fundoplication, hiatal hernia repairs) using current best practices to reduce morbidity and length of stay.
  • Perform and interpret advanced diagnostic and therapeutic endoscopic procedures related to foregut disease (endoscopic mucosal resection, endoscopic submucosal dissection familiarity preferred) and collaborate with gastroenterology on combined endo-laparoscopic approaches.
  • Manage perioperative care of complex upper GI patients, including optimization of comorbidities, nutritional assessment/intervention (enteral/parenteral nutrition), pulmonary prehabilitation, and coordination of ICU-level and step-down care postoperatively.
  • Lead and participate in multidisciplinary tumor boards and case conferences for esophageal and gastric malignancies to develop individualized, guideline-concordant treatment plans in collaboration with medical oncology, radiation oncology, gastroenterology, radiology and pathology.
  • Implement and champion enhanced recovery after surgery (ERAS) protocols specific to foregut procedures, tracking adherence and outcomes to continuously reduce complications and improve patient experience.
  • Provide acute on-call surgical coverage for upper GI emergencies (perforation, bleeding, obstruction, anastomotic leak) with the ability to triage, stabilize, and perform urgent operative or endoscopic interventions.
  • Interpret cross-sectional imaging (CT, PET-CT) and staging studies in the context of operative planning and communicate findings effectively to patients and treating teams.
  • Perform complex anastomosis techniques, leak prevention strategies and intraoperative physiologic monitoring to minimize postoperative complications and improve functional outcomes.
  • Supervise and mentor residents, fellows, advanced practice providers (NP/PA) and medical students in the evaluation and surgical management of foregut disease, providing stepwise teaching in the OR and clinic.
  • Maintain accurate procedural documentation, operative reports, and dictated clinical assessments that adhere to institutional, regulatory and billing requirements.
  • Participate in quality improvement and patient safety initiatives focused on reducing surgical site infections, pulmonary complications, readmissions, and anastomotic leak rates within the foregut service.
  • Provide long-term follow-up and survivorship care for patients after oncologic resections, including surveillance planning, nutritional support, symptom management and coordination with primary care and supportive services.
  • Engage in clinical research, clinical trials enrollment, outcomes research, and registry participation related to esophageal, gastric and foregut disease; present findings at regional or national meetings when appropriate.
  • Collaborate with administrative leaders on program development: build a comprehensive foregut center of excellence, define service lines, optimize patient pathways, and grow referral networks.
  • Manage clinic schedules and workflow effectively, including pre-op optimization clinics, post-op wound and nutrition visits, and multidisciplinary clinic sessions.
  • Ensure compliance with credentialing and privileging requirements for operative and endoscopic procedures; maintain necessary certifications (BLS, ACLS, ATLS as applicable).
  • Provide patient- and family-centered communication, delivering complex surgical risks/benefits and post-operative expectations with empathy and cultural competence.
  • Lead case reviews and morbidity & mortality conferences related to foregut surgical care, driving system-level improvements from root cause analyses.
  • Coordinate transitions of care, including discharge planning, home health services, inpatient rehabilitation, and outpatient follow-up to reduce readmission risk.
  • Develop and maintain protocols for the management of complications such as anastomotic leak, strictures, dysphagia and nutritional insufficiency, integrating interventional radiology and endoscopy as needed.
  • Participate in institutional committees related to surgical services, quality, credentialing, and education to influence strategic direction for thoracic/abdominal surgical programs.
  • Stay current on evidence-based guidelines and emerging technologies (endoscopic therapies, robotic platforms, virtual navigation) and adopt best practices that improve patient outcomes and operational efficiency.

Secondary Functions

  • Contribute to departmental and institutional research proposals, grant applications, and peer-reviewed publications to advance foregut surgical knowledge.
  • Teach and develop curriculum for residents and fellows, including simulation-based training in minimally invasive and robotic techniques.

Education & Experience

Educational Background

Minimum Education:

  • MD or DO degree from an accredited medical school and completion of an ACGME-accredited General Surgery residency (or Thoracic Surgery residency equivalent). Active medical license and ability to obtain hospital privileges.

Preferred Education:

  • Fellowship training in Upper GI/Foregut Surgery, Esophageal/Oncologic Surgery, Minimally Invasive & Robotic Surgery, or Thoracic Surgery fellowship with upper esophageal focus. Academic track record preferred for tenure-track positions.
