Core Responsibilities

Primary Functions

  • Monitor and document anthropometric and metabolic outcomes (weight, BMI, waist circumference, blood pressure, lipids, A1c) and use data-driven approaches to adjust care plans and report program-level performance outcomes against benchmarks.

Education & Experience

Educational Background

Minimum Education:

Preferred Education:

  • Registered Dietitian Nutritionist (RDN), Licensed Clinical Social Worker (LCSW), Nurse Practitioner (NP), Physician Assistant (PA), Certified Diabetes Educator (CDE), or Master’s degree in Public Health, Nutrition, Behavioral Health, or related field.
