Core Responsibilities
Primary Functions
- Develop, plan and deliver evidence-based health education programs and workshops focused on chronic disease prevention, nutrition, physical activity, mental health, stress management, tobacco cessation and other wellness topics to diverse adult and pediatric populations.
- Conduct comprehensive community needs assessments using surveys, focus groups and stakeholder interviews to identify health priorities, barriers to care and opportunities for targeted wellness programming.
- Create culturally competent curricula, lesson plans, printed materials and digital content (videos, webinars, e-learning modules and social media posts) that translate clinical guidelines into understandable, actionable health behaviors.
- Provide 1:1 health coaching and motivational interviewing to support behavior change (weight management, smoking cessation, medication adherence, physical activity goals) and develop individualized wellness plans with measurable goals.
- Organize and facilitate interactive group education sessions, support groups and workplace wellness activities that use adult learning principles to maximize engagement and retention.
- Screen and triage participants for risk factors (blood pressure, BMI, glucose, cholesterol, depression screening) and refer to clinical or community resources as appropriate; document findings and follow-up plans.
- Collaborate with clinical teams (PCPs, nurses, dietitians, behavioral health specialists) to integrate health education into care pathways and discharge planning; ensure continuity of support across care settings.
- Monitor program participation, collect performance metrics and analyze outcomes to demonstrate program effectiveness, inform improvements, and meet reporting requirements for funders and stakeholders.
- Write grant applications, proposals and program reports to secure funding and partnerships for wellness initiatives; maintain relationships with funders and community partners.
- Develop marketing and outreach strategies—including flyers, email campaigns and social media—to recruit and retain participants, increase program visibility and meet enrollment targets.
- Train and supervise peer educators, community health workers, volunteers and student interns to deliver consistent, high-quality education and outreach.
- Maintain accurate participant records, program documentation and data entry in electronic health records (EHRs), learning management systems (LMS) or program databases while ensuring HIPAA compliance and confidentiality.
- Adapt educational interventions for different learning styles, literacy levels and language needs; coordinate translation services and culturally appropriate materials.
- Coordinate health fairs, screening events and multi-stakeholder community initiatives; manage logistics, vendor relationships and on-site staffing to deliver seamless events.

- Apply behavior-change theories (Transtheoretical Model, Social Cognitive Theory, Health Belief Model) and evidence-based guidelines to design interventions that increase adherence and long-term lifestyle changes.
- Stay current with clinical guidelines, public health recommendations, and emerging wellness trends; disseminate new evidence to staff and adjust programming accordingly.
- Ensure program quality and regulatory compliance by implementing safety protocols, maintaining certifications (CPR, First Aid) and completing mandatory training modules.
- Manage program budgets, procure supplies and track expenses to maximize resources and deliver cost-effective educational services.
- Build and sustain strategic partnerships with community-based organizations, schools, employers, insurers and faith-based groups to expand program reach and service integration.
- Lead pilot projects and continuous improvement initiatives, using rapid-cycle testing (PDSA) to optimize engagement, accessibility and outcomes.
Secondary Functions
- Support data collection, reporting and continuous quality improvement activities tied to wellness initiatives, including helping to maintain dashboards and KPI trackers.
- Act as a community liaison to identify referral pathways and support cross-referrals between clinical care teams and social services.

Education & Experience
Educational Background
Minimum Education:
- Bachelor's degree in Health Education, Public Health, Nursing, Kinesiology, Social Work or related field.
Preferred Education:
- Master’s degree in Public Health (MPH), Health Education, Community Health, or related discipline.
- Certified Health Education Specialist (CHES) or Master Certified Health Education Specialist (MCHES) is highly desirable.
- Nursing license (RN) or relevant clinical certification advantageous for clinical integration roles.
